Scholarship Board Conflict-of-Interest Disclosure

All individuals serving on the It’s A Family Affair Scholarship Board should complete this form.

To maintain a fair and impartial scholarship selection process, Scholarship Board members must disclose any actual or potential relationship or circumstance that could affect—or reasonably appear to affect—their impartiality when reviewing a scholarship applicant.

A potential conflict may include a family or household relationship, close personal relationship, financial or business relationship, supervisory or mentoring relationship, or another circumstance that could create an appearance of favoritism or bias.

Disclosure of a potential conflict does not mean that a board member has done anything wrong. Disclosure helps protect the integrity of the scholarship selection process.
